Let's call her... Sarah.

Sarah is a nymphomaniac. If controlled (primarily through one partner), this isn't necessarily bad. But, Sarah doesn't really care about any one person, and just wants sex. Lots of it. So, she starts with Tom, but doesn't act as though she's in relationship. Tom thinks he is, though.
So, when he finds out that Sarah slept with Jim last night, Tom's not going to have nice names to call Sarah.

The same applies even if we drop the nymphomania, but...it puts more emphasis on the desire for sex.

There's not really a dedicated time frame/number ratio, like saying "Three in a week". "Frequent changes in partner" is probably one's best bet.
Even if it's not due to a desire for sex, I guess. One doesn't necessarily have to be aiming for anything to be given the title. Perhaps they're just having bad luck with relationships, and happen to be moving from significant other to significant other, at the moment. It's not the intention to be horribly promiscuous, but...it's what's occuring.
Granted, it doesn't mean such a person isn't a slut. Just that they may not realize that their actions are leading to the label.

What does a label matter, anyway? Sex is sex; There doesn't have to be a negative connotation associated with having a lot of it.

Depends on how old you are and how long since you stopped being a virgin.

For example, my ex was a lot older than me (not going there) and had sex with 8 people. I didn't really care.

My friend's sister is 16 and has sex with 12 people. Nuff said.

In all seriousness, I really do think with more open attitudes toward sex and less sexist viewpoints which allowed guys to be complete and utter assholes while penalizing females... the slut thing is pretty much roadkill at this point.

That however does not give license in my opinion to go out and hurt people just to get your rocks off. That's completely gay, and if anything could be derogatory or called 'slut' I'd call it that. Really though slut seems to be directed only at females, if a girl has sex with more than one guy people get the idea she's a slut, if a guy does he's well a guy. Truth is there are probably more Man Whore's than sluts any ways.

Yea but just bouncing around between partners (for either gender) gives off a lack of respect for urself and could be perceived as a slutty way of carrying yourself. I don't think it should be something so easily given away that your legs are spread for any takers.
